    The USA PATRIOT Act, a federal law, requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account, including business accounts. Park Bank associates will have to ask you for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents, as well as documents relating to the business.

    • Agrees to submit any other documentation, including but not limited to financial statements and tax returns.
    • Agrees that if this application is approved, use of the card(s) will constitute acknowledgement of receipt and agreement to the terms of the Credit Card Disclosure and Agreement and that you agree to be personally responsible, both individually and jointly with the Business, for payment of all balances incurred on all cards and accounts issued pursuant to this application now or whenever such additional accounts may be.
