Right to Receive a Copy of Appraisal: If your loan will be secured by a lien on a 1-4 unit residential dwelling, we may order an appraisal to determine the property's value and charge you for this appraisal. We will promptly give you a copy of any appraisal, even if your loan does not close. You can pay for an additional appraisal for your own use at your own cost.

  1. Warrants and represents that the information provided is true and correct and understands that Park Bank will rely upon this information to make a credit decision.
  2. Authorizes Park Bank to verify the accuracy of the information contained in this application or in conjunction with this application through additional sources as deemed necessary.
  3. Acknowledges that it may be a federal crime to knowingly make false statements concerning any of the above information under Title 18, United States Code Section 1014.
  4. Agrees to pay all third party costs without limitation that are incurred by Park Bank throughout this process. Third party costs include, but are not limited to, appraisals, credit report, flood determination, environmental, title work, etc.).
